Dr. Amber Farook, MD, is a family medicine physician who offers primary care services to patients of all ethnicities and ages. Grace Chao, PA-C is a certified physician assistant and welcomes new patients of all ages. She provides primary care services including preventative/wellness exams, chronic disease management, mental health, and has a passion for women's health and the pediatrics/adolescent population.
